Hamilton Med Spa offers a healthcare subscription service dedicated providing high quality holistic care for endocrine conditions. For a fixed monthly price, our clients gain access to a selection of top-tier practitioners focused on delivering personalized care.

Our services are available to individuals who meet the following criteria:

• At least 18 years of age.
• Residency or physical presence ion a state where our services are offered, as permitted by relevant law.
• A demonstrated medical need for treatment, which may include recent lab results for certain conditions.

Our process is streamlined into three easy steps:

• 1. Complete a comprehensive medical intake. Lab work may be necessary to confirm the medical need for treatment.
• 2. Engage in a Telehealth consultation with a healthcare provider.
• 3. Receive prescribed medication via mail, as determined suitable by your healthcare provider, on a 30, 60, or 90-day cycle.

Our goal is to make healthcare affordable. While costs vary by services and treatment options, we strive to ensure transparency with all prices before billing. At intake you should receive a Good Faith Estimate for the costs of our services. Our clients often find they save significantly compared to traditional care paths.

We do not accept insurance, cost-sharing, or similar payment methods. Clients are responsible for the service costs, which are communicated clearly and upfront.

We pride ourselves on transparency. Fees for the prescription weight loss services are a monthly subscription. We provide you with a Good Faith Estimate at intake and annually.

After completing your intake forms, our teams will arrange your Telehealth appointment with a provider, which will be conducted via a secure, HIPPA-compliant video call system.

• Each service has specific age requirements, detailed as follows:
  • GLP-1 Weight Management: 18-75 years of age.
  • Medical Weight Management: 18-70 years of age.
  • Testosterone Replacement Therapy (TRT): 30-75 years of age.
  • Insomnia Management: 18-65 years of age.

Medications are mailed directly from the pharmacy to the address on file.
